Our office will be closed from December 24th- January 1st.
- To make your payments:
- You may call the office to pay by phone by December 23rd. 435-673-9661.
- You may mail your payment in with your statement; you may also mail your credit card payment by filling out the top of your statement.
- You may pay online by going to our website www.ortho-specialists.com click on "patient log-in" on the top right of the screen. Fill out the first and last name of the patient, if you do not have a password click on "send me my password," you must have shared your email address with us for this to work. You will get a password emailed to you. Once you are logged in your can change your password.
- You man slide your payment under our door enclosed in an envelope, along with the patient name and ID number or statement.
